We’re committed to providing our certified patients with a variety of medical marijuana products engineered and cultivated to treat the medical conditions approved by the Maryland Medical Cannabis Commission (MMCC).
In an effort to service future patients, we have provided a patient registration form below. After you have completed the form below, we will contact you and provide a list of registered Allegany County providers who can certify you to purchase medical marijuana at our dispensary.
You must be 18 years of age, or under 18 with a registered caregiver to oversee your condition, with any of the below, approved medical conditions to register:
- Cachexia (A state of severe weight loss and tissue wasting secondary to underlying disease — e.g., AIDS, terminal cancer, congestive heart failure, COPD, multiple sclerosis, anorexia nervosa, malnutrition, congestive heart failure, tuberculosis, familial amyloid polyneuropathy, mercury poisoning and hormone deficiency.)
- Chronic Pain
- Post-Traumatic Stress Disorder
- Severe Nausea
- Severe or Persistent Muscle Spasms
- Severe Pain
- Wasting Syndrome
Only patients who have been certified by a registered Maryland certifier and registered patient caregivers can obtain marijuana from the Allegany Medical Marijuana Dispensary.

You may register with the Maryland Medical Cannabis Commission yourself by following the guidelines below:
To register, you will need to go to mmcc.maryland.gov then click on patient and caregivers. Please note important steps to legally obtain medical cannabis:
*Step 1: Register as a patient, or patient caregiver, with Maryland Medical Cannabis Commission (MMCC)
What you need:
- You must have a valid, accessible email account
- An electronic copy of a valid US government-issued photo ID and proof of Maryland address
- An electronic copy of a clear, recent photo
- The last four digits of your Social Security Number
You will receive an email after MMCC has reviewed your application, indicating whether or not your application has been approved.
Once approved by MMCC, you will receive an ID number that will be necessary for you to be certified for medical marijuana use.
Registering as a patient caregiver?
Minor patients are required to have at least one caregiver assigned to their accounts at all times. Only parents or legal guardians 21 years of age and older are eligible to serve as caregivers for minor patients.
The caregiver is also responsible for the doctor’s approval to become a caregiver of a patient. Caregivers must register with the Commission prior to registering the minor patient. Registering a minor patient requires additional forms, one of which requires notarization, and the caregiver must have a valid, accessible email account.
You will need electronic copies of:
- Caregiver’s valid US government-issued photo ID
- Clear, recent photo of minor patient
- Proof of caregiver’s relationship to patient (birth or adoption certification or court order)
- Notarized Minor Patient Form
- The last four digits of the patient’s Social Security Numbers, as well as the Caregiver’s ID number
Once application is submitted, the MMCC must review and approve the caregiver’s application. Once approved, caregiver registration is valid for two years.
Purchase ID card: Before visiting a licensed Maryland dispensary, the registered caregiver, after the patient is certified must purchase a Caregiver ID card for one-time fee of $50.
Step 2: Obtain a certification from a provider who is registered with the Commission
Medical cannabis cannot be purchased without a valid certification from a certifier registered with MMCC.
Certifications are valid for a maximum of one year.
Note: If nothing is dispensed from a certification within 120 days of issue, the certification expires and you will need to obtain a new certification from your certifier.
Step 3: Visit Allegany Medical Marijuana Dispensary
After your registration has been approved by MMCC and your registered provider has certified you, you are ready to visit a licensed Maryland dispensary to purchase medical cannabis.
Step 4: Purchase ID card (optional with a one-time fee of $50)
ID cards are not required. They are optional for registered patients and cost a one-time fee of $50. Only patients who have obtained a valid certification from a registered provider are eligible to purchase an ID card from MMCC, if they choose.
Patients from Maryland who have been certified for medical marijuana use by a registered Maryland provider can obtain medical marijuana from the Allegany Medical Marijuana Dispensary.
Out of State
Allegany Medical Marijuana Dispensary encourages out-of-state patients and out-of-state patient caregivers, to register with our dispensary. The State of Maryland is in the process of establishing criteria for out-of-state patients to complete the State of Maryland registration process and acquire a Maryland Patient ID number required for certification. The State of Maryland hopes to have a final policy resolution in the near future. By reregistering with our dispensary, you will be kept up to date on important issues through regular emails.
Once you register and your registration has been approved by MMCC, and your provider has completed your certification, you are ready to visit Allegany Medical Marijuana Dispensary to purchase medical cannabis.
After you are certified, Allegany Medical Marijuana Dispensary will offer free consultations based on your medical condition. Our trained consultants will meet with our patients using PotBotics iPad software to determine the best product for the condition. At this time, budtenders and patients, or patient caregivers, may discuss and document some of the following topics: medication history, treatment goals, strain/product selection, and any additional questions.
Medical Cannabis Card
ID cards are optional for patients and cost a one-time fee of $50. (A card is not required to purchase medical cannabis.) An ID card cannot be purchased until a valid certification has been obtained from a certifier registered with MMCC. However, this is required for patient caregivers. Before visiting a licensed Maryland dispensary, the registered caregiver, after the patient’s certification is received must purchase a Caregiver ID card for one-time fee of $50.
Please note: A medical cannabis card may be helpful and demonstrates that the Maryland Cannabis Commission recognized that the cardholder is a registered patient or caregiver and qualifies to legally purchase medical cannabis in the state of Maryland.
Allegany Medical Marijuana Dispensary cannot guarantee that having a medical cannabis card will protect the patient, or patient caregiver, from law enforcement.

Medical Provider Registration
If you are a Maryland MD of any type – a Nurse Practitioner, Nurse Midwife, Dentist or Podiatrist – the Maryland Medical Cannabis Commission encourages you to learn about the benefits of medical cannabis and register with the MMCC as a provider to certify qualifying patients for medical marijuana use.
Providers can limit their certification to only patients they treat or a provider may establish a certification practice. This link will guide you through the state of Maryland registration process.
